About The Position

We are hiring a Certified Nursing Assistant for a Hospice position providing care to patients in their homes in the Wexford, PA, area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The CNA will provide hands-on care, assist with activities of daily living (ADLs), observe and report changes in condition, and support patients and caregivers by reinforcing the care plan and offering emotional support. Responsibilities include responding promptly to patient needs, accurately documenting care in the EMR, and traveling to patient homes within an assigned territory.

Requirements

• Active Certified Nursing Assistant (CNA) certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• Legally authorized to work in the United States

• Hospice experience preferred

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Travel to patient homes within assigned territory
