About The Position

Under the leadership of the Director of Hospice, the Hospice Case Manager is an active member of the Hospice IDG team that delivers professional end-of-life care and services consistent with the strategic vision, goals, philosophy and direction of the Hospice Department and CRHS. The Hospice Case Manager reviews patient and family needs and develops age-appropriate and culturally sensitive plan of care with goals mutually set by the IDG team and the patient and/or caregiver.

Requirements

  • Associate’s degree in nursing
  • One to two years of nursing experience
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Maintains a current certification in Basic Life Support (If BLS verification is not current upon hire, it must be obtained within three (3) months of hire.)
  • Maintains a current valid Driver’s License and proof of minimum automobile insurance.
  • A Motor Vehicle Report is required upon hire and reviewed annually.
  • Self-directed and able to work with minimal supervision.
  • Shows ability to organize and prioritize workload independently.

Nice To Haves

  • A Bachelor’s degree is preferred.
  • prefers experience in hospice, med-surge, oncology, ICU or ED.

Responsibilities

  • Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness (es).
  • Assists the IDG team in developing a plan of care based on the goals of the patient and family/caregiver. Coordinates the plan of care with the patient, family/caregiver and IDG team.
  • Provides emotional support and education to patients and their families.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward dignity and optional comfort care.
  • Supervises the care rendered by the Hospice Aid within the patient’s home and ensures compliance with the established plan of care.
  • Ensures the compliance with established infection control procedures by all caregivers and/or family members.
  • Ensures the accurate documentation of assessments, care plans, goals, medications, communications, and interventions.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
  • Actively participates in Hospice interdisciplinary group meetings.
  • Complies with Hospice quality, satisfaction and regulatory compliance standards to ensure they are either met or exceeded.
  • Mentors new employees as requested.
  • Completes all required professional education including continuing nursing education hours and mandatory in-services to maintain professional licensure.
  • Participates in on-call duties as defined by the on-call policy.
  • Performs other related job duties as assigned.
