About The Position

Hospice RN Case Manager We give nurses the time they need to truly care for patients and families at the most important moments. Mon-Fri 8-5p, and 1 weekend a month. no on call. Covering Fresno $10,000 Sign on bonus 2026 mileage reimbursement for approved business-related travel at $.725/mile Smaller caseloads More time with patients Less burnout than hospital roles The Hospice RN Case Manager is responsible for delivering comprehensive, patient-centered nursing care to hospice clients. This role serves as the primary coordinator of care, ensuring patients receive appropriate symptom management, emotional support, and end-of-life care aligned with physician orders and hospice plans.

Requirements

  • RN license in state of practice
  • Graduate of accredited nursing program
  • CPR certification
  • Minimum 1 year acute care nursing experience
  • Valid driver’s license and reliable transportation
  • California-specific: 1 year professional nursing experience required
  • Clinical judgment and critical thinking
  • Compassion and emotional intelligence
  • Strong communication and teaching ability
  • Team collaboration and leadership
  • Time management and adaptability

  • Hospice/home health experience preferred

Responsibilities

  • Perform patient assessments, develop and update individualized care plans
  • Manage overall patient care, serving as the designated case manager
  • Monitor patient condition and communicate changes to physicians and care team
  • Provide symptom management using evidence-based hospice/palliative practices
  • Deliver education to patients and families on disease progression, self-care, and end-of-life expectations
  • Collaborate with interdisciplinary team (IDT/IDG), including physicians, aides, and other clinicians
  • Coordinate referrals and services to ensure timely, efficient care
  • Lead case conferences and contribute to care planning discussions
  • Provide functional supervision to LPNs and Hospice Aides
  • Conduct supervisory visits (bi-weekly for aides, monthly for LPNs)
  • Delegate tasks appropriately based on skill level
  • Participate in training, orientation, and mentoring of staff
  • Maintain accurate, timely clinical documentation (assessments, care plans, visits, physician communications)
  • Ensure compliance with federal/state regulations, hospice standards, and agency policies
  • Uphold confidentiality and HIPAA standards
  • Address grief, loss, and emotional needs of patients and caregivers
  • Provide anticipatory guidance around the dying process
  • Promote dignity, comfort, and quality of life
