About The Position

We’re hiring a Hospice Case Manager (RN) who wants a predictable weekday schedule, meaningful patient relationships, and the autonomy to provide compassionate care in patients’ homes. You’ll step into a field-based hospice role providing case management throughout Gwinnett County. You’ll conduct assessments, develop care plans, manage symptoms, educate patients and families, and coordinate care while working alongside a supportive interdisciplinary hospice team. This role offers a Monday through Friday schedule with a shared on-call rotation, averaging approximately 4 patient visits per day. It is a home-based field position with significant clinical autonomy within a growing hospice program with interdisciplinary team support.

Requirements

Active Georgia RN license
Current CPR certification
At least 1 year of nursing experience
Comfortable providing independent, compassionate care in patients’ homes
Valid driver’s license and reliable, insured transportation
Value patient-centered end-of-life care and supporting patients and families

Nice To Haves

Hospice experience preferred
Strong RNs from Home Health, ICU, CCU, Oncology, or Med-Surg backgrounds
