About The Position

We’re hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, schedule stability, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance. You’ll step into a field-based hospice role providing direct patient care in homes and care facilities throughout Cape Cod. You’ll independently manage your patient caseload while collaborating with an experienced interdisciplinary team dedicated to providing exceptional end-of-life care. Your territory will include Barnstable, Bourne, Cape Heritage, Cape Regency, and surrounding Cape Cod communities.

Requirements

• Active Massachusetts RN license
• At least one year of nursing experience
• Comfortable independently managing a field-based caseload
• Works well as part of an interdisciplinary healthcare team
• Comfortable traveling throughout the Cape Cod territory

Nice To Haves

• Hospice, home health, ICU, or oncology experience is preferred
• Value compassionate, patient-centered end-of-life care
