About The Position

We're hiring a Hospice Case Manager (RN) who wants meaningful one-on-one patient relationships, schedule stability, and the opportunity to provide compassionate end-of-life care while maintaining an excellent work-life balance. You'll step into a hospice field role providing direct patient care in homes and care facilities. You'll manage your own patient caseload while collaborating with an experienced interdisciplinary team dedicated to delivering exceptional hospice services. You'll practice throughout the Warwick, Rhode Island area, a coastal community offering beautiful waterfronts, historic neighborhoods, and convenient access to Providence.

Requirements

• Active Rhode Island RN license or a Compact RN license eligible for Rhode Island
• At least one year of nursing experience
• Value compassionate, patient-centered care
• Comfortable working independently while collaborating with an interdisciplinary team

Nice To Haves

• Hospice or home health experience is strongly preferred
