About The Position
BAYADA Hospice is seeking a compassionate and dedicated Full Time Registered Nurse (RN) to join their team as a Hospice Case Manager in the Brattleboro, VT office. This role involves servicing hospice clients on a per visit basis in the Ludlow, Springfield, Chester, Bellows Falls, and Claremont areas of VT and NH. As a hospice nurse, the RN will be an integral member of a multi-disciplinary health care team, providing palliative care to clients and enabling them to receive necessary medical care at home. Requirements
- A current license as a Registered Nurse (RN) in VT & NH
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Ability to travel to cases as assigned
Nice To Haves
- Prior home care and/or hospice experience strongly preferred
- Prior experience with electronic medical records (EMR) preferred
Responsibilities
- Making home visits to clients in designated geographic territories
- Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care
- Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager
- Following up with, executing, and properly documenting doctors' orders
- Performing client assessments as necessary
- Occasional on call after hours and/or weekends
