About The Position

We are seeking an experienced Hospice Business Development Liaison who understands the hospice landscape and has a proven track record of building referral relationships and driving admissions. This is not an entry-level opportunity. The ideal candidate brings existing provider relationships, strong market knowledge, and the ability to independently grow a hospice census. This role is designed for a seasoned hospice liaison who wants the autonomy to grow a territory and make a direct impact on patients and families through increased access to end-of-life care.

Requirements

  • Proven experience in hospice or home health business development/liaison role
  • Demonstrated success in meeting or exceeding admission/census goals
  • Deep understanding of hospice services, regulations, and referral processes
  • Highly self-directed, with the ability to manage a territory independently
  • Valid driver’s license and auto insurance

Responsibilities

  • Leverage existing relationships with physicians, hospitals, SNFs, and community partners to drive referrals
  • Actively grow hospice admissions through strategic business development efforts
  • Position Choice as the preferred hospice provider in the market
  • Educate referral sources on hospice eligibility, services, and benefits
  • Collaborate with clinical teams to ensure seamless patient transitions
  • Execute targeted outreach and community engagement strategies
  • Consistently meet or exceed monthly admission goals
