About The Position

Unified Care Hospice is seeking a compassionate and dependable Bilingual Certified Nursing Assistant (CNA) to join our growing clinical team. In this role, you will provide hands-on personal care and emotional support to patients and families in their homes and care facilities. Positions are available for Full-Time, Part-Time, and PRN (as-needed) schedules. Bilingual (English/Spanish) candidates are strongly preferred to better serve our diverse community.

Requirements

Active CNA certification in the State of Texas
Current CPR certification
Valid driver’s license, reliable transportation, and active auto insurance
Strong professionalism, reliability, and time management skills
Ability to work independently and within an interdisciplinary team

Nice To Haves

Bilingual (English/Spanish) preferred
Prior hospice, home health, or long-term care experience preferred
Familiarity with EMR systems
Experience with dementia, behavioral health, or end-of-life care is a plus

Responsibilities

Provide personal care including bathing, grooming, dressing, toileting, and feeding
Support patient comfort, safety, and dignity at all times
Monitor and document patient condition and vital signs in the EMR
Follow the individualized Plan of Care under RN supervision
Assist with safe transfers and mobility using appropriate equipment
Maintain infection control and safety standards
Communicate clearly with patients, families, nurses, and leadership
Complete daily/same-day charting per agency policy
Deliver supplies and assist with basic patient support as needed
Participate in required trainings and in-service education
