About The Position

Serenity Hospice Care is seeking an enthusiastic and organized Bereavement Coordinator for its Springfield, New Jersey office. The position provides bereavement services and support to patients’ families and caregivers, hospice staff, volunteers, and the wider community. The coordinator makes scheduled bereavement calls throughout the 13-month bereavement period, coordinates care with the Interdisciplinary Team, supports program development and evaluation, and provides grief support and training. The role is full time, Monday through Friday, from 8:30 a.m. to 4:30 p.m.

Requirements

At least two years of hospice experience.
Excellent computer skills, especially Microsoft Office.
Ability to be flexible and adapt to changes as needed.
Ability to self-manage and work with teams.
Organized and detail-oriented.
Reliable and trustworthy.
Driver’s license and reliable transportation.

Responsibilities

Make bereavement calls throughout the 13-month bereavement period.
Make moderate-risk calls at 6 and 12 months and high-risk calls monthly, according to schedule.
Plan, implement, and maintain services for patients, identified caregivers, hospice staff, and the community.
Update bereavement plans of care as necessary.
Attend Interdisciplinary Team meetings and participate in developing patient, family, and caregiver bereavement and comprehensive plans of care.
Coordinate with the Interdisciplinary Team regarding family and caregiver bereavement needs.
Plan bereavement program development, expansion, and refinement through annual evaluation of services.
Provide one-to-one bereavement support to hospice personnel coping with work-related grief and refer them to community resources.
Conduct specialized bereavement training and oversee volunteers providing bereavement services.
Stay current with bereavement and grief counseling trends and knowledge.
Perform other duties as assigned.
