AdvisaCare Hospice and Palliative Care Program is seeking a Nurse Practitioner to join their family. The program is looking for someone who holds a universal mission to provide compassionate care to all who need their services. This role involves providing medical evaluations for predominantly elderly, frail, and seriously ill patients in their home environment, ordering and interpreting tests, prescribing medications, and ordering treatments and durable medical equipment. The Nurse Practitioner will also assist patients and families in identifying goals of treatment and ongoing plans of care, communicating these goals to the Interdisciplinary Team, and consulting with the palliative care physician as needed. Responsibilities include preparing and maintaining accurate patient records, serving as a liaison between patients and the multidisciplinary care team, participating in Quality Assurance activities, and complying with applicable laws and regulations. The role also involves providing training and education to staff, defining goals for professional growth, and engaging in self-care activities. Collaboration and communication with the Medical Director, attending physician, hospital staff, and the Interdisciplinary Team are crucial, including participation in bi-weekly team meetings. The Nurse Practitioner will educate patients and caregivers on disease processes, goal setting, symptom control, treatment options, prognosis, and advance care planning. Additionally, the role includes supervising nurse trainees, operating an automobile for home visits, and the ability to lift/transfer/push/pull up to 100 pounds infrequently. The position requires the ability to complete multi-tasks and meet deadlines, and proactively promote census growth by offering care that meets the diverse needs of the community.
