About The Position

AdvisaCare Hospice and Palliative Care Program is seeking a Nurse Practitioner to join their family. The ideal candidate will share the universal mission of providing compassionate care to all who need their services. This role involves conducting medical evaluations of elderly, frail, and seriously ill patients in their home environment, ordering and interpreting tests, prescribing medications, and developing treatment plans. The Nurse Practitioner will also play a key role in care coordination, patient and family education, and collaborating with the Interdisciplinary Team and Medical Director. The position requires maintaining accurate patient records, participating in quality assurance, and providing education and onboarding support for staff. Additionally, the role involves supervising nurse trainees and promoting census growth.

Requirements

  • Current unrestricted Nurse Practitioner in Michigan required.
  • Excellent interpersonal skills, sound judgement, effective organizational, prioritization and follow-through skills, attention to detail, tact, dependability, emotional intelligence, ability to maintain confidentiality and promote positive, constructive relationships with communication and collaboration at all levels required.

  • Meets health requirements demanded of all clinical/field staff who provide service through the agency.
  • Must be able to operate an automobile/has access to personal transportation.
  • Able to lift/transfer/push/pull up to 100 pounds infrequently using assistive techniques and devices.

Nice To Haves

  • Minimum of one-year experience in a Medicare-certified home health/hospice agency preferred.
  • Demonstrated knowledge of home health nursing/hospice and/or outpatient services preferred.
  • Willing and able to meet the agency need to support hospice Medical Director/designee.
  • Understanding of performance improvement with the ability to communicate and operationalize performance improvement initiatives at the departmental and organizational level.

Responsibilities

  • Performs routine and urgent assessments, including medical evaluations of patients in their home environment.
  • Orders, performs, and interprets laboratory and radiology tests.
  • Prescribes medications, including controlled substances, within the scope of practice.
  • Orders treatments and durable medical equipment.
  • Assists patients and families in identifying goals of treatment and ongoing plans of care.
  • Communicates patient goals among the Interdisciplinary Team.
  • Consults with palliative care physician or designees as needed.
  • Assists in care coordination for palliative care referrals.
  • Prepares and maintains accurate patient records, charts, and documents.
  • Serves as a liaison between patient and other members of the multidisciplinary care team.
  • Participates in Quality Assurance activities.
  • Complies with applicable laws and regulations.
  • Provides training and ongoing education and onboarding support for staff.
  • Defines goals for professional growth and participates in professional activities.
  • Engages in active and frequent self-care activities.
  • Adheres to the practice of confidentiality.
  • Collaborates and communicates with Medical Director, attending Physician, hospital staff, and Interdisciplinary Team.
  • Participates in Interdisciplinary Team meetings.
  • Completes and submits required clinical documentation within 24 hours of visit completion.
  • Educates patient and caregiver regarding care, disease process, goal setting, symptom control, treatment options, prognosis, and Advance Care Planning.
  • Supervises nurse trainees, including graduate and advanced practice nurses in field placements.
  • Must be able to operate an automobile/has access to personal transportation.

  • Proactively promotes census growth and seeks to offer care that meets the unmet diverse needs of the community.
