About The Position

The Hospice and Palliative Care Hospital Liaison will develop, maintain, and grow hospital relationships to cultivate collaboration and partnerships with Tri-Cities Chaplaincy in Benton and Franklin Counties, as well as surrounding communities.

Requirements

  • A bachelor’s or master’s degree in divinity, theology, psychology, or a related discipline from an accredited school of higher education.
  • Ability to manage a patient caseload and work independently and as part of an interdisciplinary team.
  • Expertise in written and oral communication and competence with electronic medical record (EMR) documentation.
  • Ability to quickly adapt to changing situations and respond accordingly.
  • Ability to provide hospice education to facility staff members, patients, and families.
  • Willingness to travel within the community.
  • Availability to work flexible hours, including evenings, weekends, and holidays as needed for chaplain on-call rotation support.
  • Reliable transportation and the ability to independently drive within Tri-Cities Chaplaincy’s service areas to provide in-home patient and family services and support in clients' homes, facilities, and community locations.
  • Must possess and maintain a valid driver’s license, as driving is an essential function of this role.
  • Must meet insurance eligibility requirements.
  • This position is designated as Safety Sensitive.
  • Must be able to pass a pre-employment drug screen which includes cannabis testing in Washington State.
  • Washington State Food Worker Card issued by the Department of Health (for Hospice House).
  • Ability to report to assigned work location(s) as scheduled and maintain consistent, reliable attendance.
  • Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
  • Maintain reliable personal transportation.
  • Maintain a valid driver’s license and vehicle insurance per state requirements.
  • Satisfy state and agency health requirements by providing proof of immunization, declinations, or approved accommodations as applicable.

Nice To Haves

  • Chaplaincy experience in hospice, palliative care, hospital, or crisis ministry settings.
  • Experience in end-of-life care and crisis intervention.
  • Completion of Critical Incident Stress Management (CISM) training.

Responsibilities

  • Develop, maintain, and grow hospital relationships to cultivate collaboration and partnerships.
  • Provide hospice education to facility staff members, patients, and families.
  • Manage a patient caseload and work independently and as part of an interdisciplinary team.
  • Respond accordingly to changing situations.
  • Participate in education and outreach activities to promote agency programs, including hospice care, advanced planning, and bereavement services.
  • Maintain knowledge of and comply with agency policies, procedures, and practices.
  • Protect patient and agency information per HIPAA guidelines. Only access information on a need-to-know basis.
  • Complete agency and departmental orientation and required training programs.
  • Complete ongoing education, policy reviews, and training by designated deadlines.
  • Maintain current licensure(s) and certifications(s) required for the role.
  • Read, write, speak, and understand English fluently.
  • Be available to work as assigned or scheduled.
