About The Position
VNA of Ohio is seeking PRN Hospice & Home Health Aides to join their team in Cleveland/Northeast Ohio. This role offers flexible pay options (per-visit or hourly) and flexible scheduling. Requirements
- Active STNA certification (Ohio)
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
- Genuine compassion and comfort working with seriously ill, elderly, or end-of-life patients
- Ability to follow a plan of care and work under the supervision of a licensed nurse or therapist
Nice To Haves
- Prior home health or hospice aide experience
- Experience providing personal care in a home-based or community setting
- Familiarity with HCHB or similar home health/hospice EMR
- Comfort supporting patients and families through serious illness or end-of-life care
Responsibilities
- Provide personal care assistance to hospice and home health patients in their homes including bathing, grooming, dressing, and hygiene
- Assist patients with ambulation, transfers, and mobility as directed by the care plan
- Support patients with light housekeeping, meal preparation, and errands as outlined in the plan of care
- Observe and report changes in patient condition, mood, or behavior to the supervising nurse or therapist
- Provide companionship and emotional support to patients and families
- Assist hospice patients and families with comfort-focused care needs
- Maintain accurate and timely documentation in the EMR within required timeframes
- Follow all infection control, safety, and agency policies during every visit
