About The Position

It enhances our clinical excellence with advanced, easy-to-use remote monitoring technology and 24/7 access to our Virtual Care Center RNs. Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.

Requirements

Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
Must be certified as HHA in state of California
Documentation of one year’s employment as an aide in an institution or home care setting.
Meets Saint Agnes Home Health and Hospice's hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
Must have current driver’s license and reliable transportation to and from work site

Responsibilities

Provide one-to-one care with your patients in their homes
Follow physician orders
Comply with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
Function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency’s client and their family.
Accept responsibility for maintaining skills, learning and adhering to agency's policies and procedures on an on-going basis.
