The Hospice Aide provides care ordered by the physician as indicated by the plan of care to all patients on caseload including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding, and transferring patient. They take accurate vital signs and describe the activities of daily living, demonstrating the ability to use techniques to assist the patient to meet these activities. The Hospice Aide participates in case conferences (IDG’s) to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, and new problems. They notify appropriate licensed staff when a patient complains of pain, position patients in bed with proper body alignment, and assist patients with ambulation as necessary. The role involves performing aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. The Hospice Aide supports cultural practices as long as these practices do not harm the patient/family or interfere with the planned course of medical treatment. They identify and communicate findings to the Case Manager and/or Clinical Manager, and participate in the Agency’s other mandated activities as requested by the Clinical Manager or Director of Patient Care Services (DPCS). The position requires clear, concise documentation in the medical record as it relates to reimbursement guidelines and Agency policy and procedure. The Hospice Aide identifies and communicates findings to the supervising RN and works as part of the interdisciplinary group.
