About The Position

The Hospice Aide provides personal care to clients in their homes, promoting comfort and meeting their needs according to established policies and regulations. This role involves direct patient care, including hygiene, mobility assistance, meal preparation, and maintaining a clean living environment. The Hospice Aide works under the supervision of an RN Case Manager and is responsible for observing, documenting, and reporting client status.

Requirements

  • High school diploma or GED.
  • Home Health Aide Certification training.
  • Minimum of 12 months as an aide in a facility or home health/hospice agency or acute care setting.

Nice To Haves

  • STNA strongly preferred.
  • Hospice experience preferred.

Responsibilities

  • Performing and documenting all duties assigned by the RN Case Manager.
  • Observing, recording, and reporting the client’s care, symptoms, emotional and physical status.
  • Providing caring, compassionate support to client, family members and/or caregivers.
  • Providing personal care including bathing, back rubs, oral hygiene, hair care, skin care, assisting client with dressing and undressing and assisting the patient with toileting activities.
  • Processing client’s laundry, as appropriate, changing bed linen and keeping client’s living area clean and orderly, as appropriate.
  • Planning and preparing nutritious meals and assisting in feeding the client, if necessary.
  • Taking vital signs when ordered.
  • Assisting in ambulation and transfers as appropriate to plan of care.
  • Performing range of motion and other simple procedures as an extension of therapy service as ordered.
  • Meeting safety needs of clients and using equipment safely and properly.
