About The Position
Hospice Aides provide personal care and designated health related services in compliance with policies and procedures, applicable health care standards, and government laws and regulations.
Requirements
- High School Diploma
- Active CNA in the State of IL or completion of 1st semester of RN Program
- Automobile Insurance - Sarah Bush Lincoln
- Driver's License - State of Illinois
Responsibilities
- Assists in the implementation of the patient Plan of Care.
- Assists patients with activities of daily living.
- Attends in-services, staff meetings, and continuing education.
- Follows verbal and written directions provided by the RN, specific to the patient assigned.
- Maintains a clean and safe environment and performs light housekeeping assignments as assigned.
- Maintains and submits documentation as required. Records observations, completed, and refused tasks.
- Observes, records, and reports significant changes in the status of a patient and/or family to supervisor.
- Participates in interdisciplinary team meetings.
- Prepares light meals if assigned in patient plan of care.
- Provides hands-on personal care to patients.
- Provides personal care and supportive services to hospice patients.
