About The Position

Provide compassionate, patient-centered personal care including bathing, grooming, dressing, toileting, and feeding in accordance with the hospice aide plan of care. Travel to patients’ residences, including private homes and facility settings, to deliver hospice services in a respectful and professional manner. Offer emotional support and companionship to patients and families, including spending quality companionship time to promote comfort, dignity, and quality of life. Assist patients with ambulation, transfers, positioning, and range of motion exercises as documented in the hospice aide plan of care. Provide bedside vigil support as needed during end-of-life care, ensuring patient comfort and presence during critical moments. Report any changes in the patient's condition to the registered nurse case manager. Provide respite for the patient's family/caregiver when on-site, as appropriate Sit alongside patients during their active departure. Provide support to the patient's family Perform postmortem care Use durable medical equipment safely Other duties as assigned

Requirements

  • State Tested Nursing Assistant in the state of practice (STNA)
  • CPR/BLS Certified
  • Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
  • Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
  • Must be willing to travel
  • Valid driver's license and Auto insurance Coverage with reliable transportation
