Hospice Aide CNA/HHA

Requirements
- Current Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification
- Current Long-Term Care (LTC) certification (if required by state)
- Current CPR Certification
- Valid Driver's License
- Current Auto Insurance
- Reliable transportation
- Ability to travel to patient homes within the assigned territory

- HomeCare HomeBase experience
- Previous hospice or home health experience
Responsibilities
- Provide compassionate personal care to hospice patients under the direction of the Registered Nurse and according to the individualized Plan of Care.
- Assist patients with bathing, dressing, grooming, toileting, and other activities of daily living while preserving dignity and independence.
- Safely assist patients with transfers, ambulation, and mobility according to agency policies.
- Observe, report, and document changes in patient condition to ensure continuity and quality of care.
- Maintain accurate and timely documentation of services provided.
- Practice infection prevention and maintain a clean, safe, and comfortable environment for patients.
- Assist patients with nutrition, hydration, and meal preparation as appropriate.
- Communicate respectfully and compassionately with patients, families, and members of the interdisciplinary care team.
- Protect patient privacy, confidentiality, and personal property at all times.
- Perform assigned treatments and procedures in accordance with agency policies.
- Support the mission, values, and philosophy of Choice Hospice while contributing to exceptional patient care.
