About The Position

In this position you will perform duties essential to personal care given to terminally ill persons in their place of residence. You will maintain the patient's personal comfort under the direction of the registered nurse and/or supervisor. This position reports to the Director of Clinical Services/Patient Care Coordinator.

Requirements

  • Adheres to dress code; appearance is neat and clean.
  • Completes annual educational requirements as assigned.
  • Maintains patient confidentiality at all times.
  • Reports to work on time and as scheduled, completes work within the designated time.
  • Wears identification when in community.
  • Communicates the mission, ethics, and goals of the organization.
  • High School Diploma or G.E.D.
  • Completion of a Hospice Aide certified by State Department of Health Services and is current and listed in good standing on the State Nurse Aide Registry on hire and at time of renewal or have completed a classroom and supervised practical training of at least 75 hours and provide documentation of such training.
  • Current TB screening.
  • Travel is required throughout the workday.
  • Valid state driver's license and reliable automobile, current automobile insurance and willingness to operate personal car necessitated by nature of job.
  • Provides copies upon renewal of driver's license and automobile insurance.
  • Able to communicate effectively in English, both verbally and in writing.
  • The position requires sitting, standing, walking, bending, kneeling, and stooping, crouching, reaching, and climbing, use of telephone as well as computer throughout most of the day.
  • The employee must have adequate (with correction) close vision, distance vision, depth perception, and the ability to focus.
  • The employee must frequently lift or move objects and patients weighing over 50 pounds.
  • Routinely transfer patient weight 1001b or greater.

Responsibilities

  • Follows the aide care plan and notifies supervising RN with any changes.
  • Notifies appropriate licensed staff when patient complains of pain.
  • Positions patient in bed with proper body alignment.
  • Ensures an environment that optimizes patient safety and quality care.
  • Supports and maintains a culture of safety, rights, dignity, and quality of care for all patients.
  • Demonstrates the ability to document patient care according to professional standards.
  • Works cooperatively with the Interdisciplinary Group.
  • Understands Hospice philosophy and issues of death and dying.
  • Maintain the dignity of dying patients.
  • Adheres to all organization policies and procedures.
  • Attends mandatory staff meetings.
  • Completes Documentation within 48 hours.
  • Signs in and out of facilities and documents in appropriate chart(s).
  • Maintains a good relationship with facilities and reports to appropriate staff.
  • Participates in on call coverage when needed.
  • Represents the organization in a positive and professional manner in the community.
  • Collaborate with other team members to determine attendance of funerals during work hours.
  • Other duties, responsibilities, or activities may change with or without notice.
