About The Position

At Elara Caring, the Hospice Aide helps deliver home care in keeping with the company’s mission to provide the Right Care, at the Right Time, in the Right Place. Working under the direction of a Registered Nurse, the aide provides hands-on personal care and supports patients with daily activities, mobility, safety, and comfort in their homes. The role also includes observing and reporting changes in patient condition, following individualized care plans and infection-control procedures, maintaining accurate documentation and patient privacy, and participating in required training.

Requirements

• Current state Home Health Aide or Certified Nursing Aide certification.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients walk and transfer safely and use mobility equipment.
• Follow each patient’s individualized care plan.
• Monitor and report changes in a patient’s condition to the care team.
• Help maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond appropriately to emergencies and follow safety and infection-control guidelines.
• Maintain accurate documentation and protect patient privacy.
• Participate in required training.
