Hospice Aide/ CNA
About The Position
At Grace Hospice, we provide professional care with a personal touch. Our nonprofit organization is committed to supporting patients and families through compassionate physical, emotional, and spiritual care. We are seeking a caring and dependable Hospice Aide to join our interdisciplinary team and help individuals receiving hospice care maintain comfort, dignity, and the best possible quality of life wherever they call home. Under the supervision of the Hospice Aide Manager and RN Case Manager, the Hospice Aide provides personal care, comfort, and support to patients and families during an important time in their lives.
Requirements
- Current Minnesota Nursing Assistant Certification and active status on the Minnesota Nursing Assistant Registry
- Valid Minnesota driver’s license
- Daily access to a reliable and insured vehicle
- Ability to use or learn electronic charting on a tablet or mobile phone
- Strong communication and interpersonal skills
- A compassionate approach and a desire to support patients and families receiving hospice care
Nice To Haves
- At least one year of experience as a nursing assistant or home health aide preferred
Responsibilities
- Assisting with bathing, dressing, grooming, oral care, skin care, and incontinence care
- Assisting with ambulation, transfers, repositioning, and mobility
- Obtaining and documenting vital signs
- Providing light housekeeping and other support outlined in the patient’s care plan
- Completing required documentation accurately and on time
- Communicating changes in a patient’s condition to the appropriate clinical team member
- Maintaining patient confidentiality and following HIPAA requirements
- Treating patients, families, coworkers, and community partners with kindness and respect
