About The Position
At Choice Hospice, our Hospice Aides are a vital part of the care team, providing compassionate support that helps patients remain comfortable and cared for in their homes. We are seeking caring and dependable PRN Hospice Aides who are passionate about making a difference in the lives of patients and families. As a PRN Hospice Aide, you will provide direct patient care under the supervision of an RN, assisting with personal care, mobility, nutrition, and maintaining a safe and comfortable environment. You will also communicate changes in patient condition and support patients and families with dignity, kindness, and respect.
Requirements
- Current CNA/HHA and LTC certifications
- Valid Driver’s License and Auto Insurance
- Current CPR Certification
Nice To Haves
- HomeCare HomeBase experience preferred
Responsibilities
- Assist patients with personal care and daily living activities
- Support safe transfers and ambulation
- Observe and report changes in patient condition
- Maintain accurate documentation and continuity of care
- Follow infection control and safety procedures
- Promote comfort, privacy, and dignity for every patient
