About The Position

This role is for a Hospice Aide II within the Hospice Care Team, dedicated to supporting patients and families during vulnerable moments. The Hospice Aide II serves as a trusted resource for nurses, physicians, caregivers, and loved ones, helping maintain comfort, dignity, and continuity of care. This position is ideal for someone compassionate and confident working independently in a field-based or home-care setting, with a passion for supporting colleagues, providing calm reassurance, and upholding high standards of patient-centered care. The Hospice Aide II position is an advanced role that provides direct patient care to Hospice of Cincinnati patients under the direction of a nurse. This position assists with or performs basic activities of daily living, including bathing and feeding, and provides companionship and emotional support. The Hospice Aide II is responsible for knowing safety policies and following approved procedures. In addition to standard hospice aide responsibilities, the Hospice Aide II role will also include one of the following: A. Unit Coordinator for an assigned unit and shift, performing duties of both roles. B. Cross-training to function in multiple hospice care settings, including inpatient units, home care, long-term care, assisted living, and continuous care. C. Precepting and mentorship support for new hospice aides.

Requirements

  • High School Diploma or GED
  • 2 years high school education in general studies (Required)
  • 2 - 5 years of Previous Healthcare Experience (Required)
  • Age 17 with work permit
  • Reading and math ability appropriate to the position.
  • Multi-line phone skills, familiar with patient call systems and pagers.
  • Basic Life Support (BLS) within 30 Days Required

Responsibilities

  • Provides direct patient care to Hospice of Cincinnati patients under the direction of a nurse.
  • Assists with and/or performs basic activities of daily living including bathing, feeding, and assistance with ambulation.
  • Serves as a provider of companionship and emotional support, even during times of crisis.
  • Maintains knowledge of safety policies and follows approved procedures.
  • May serve as Unit Coordinator for assigned unit and shift, performing clerical duties, data entry into the electronic medical record, coordinating patient flow, and providing telecommunication support.
  • May provide hospice aide services across multiple settings or levels of care, including inpatient hospice units, home care/long term care/assisted living, and continuous care, adapting personal care delivery to the environment, patient acuity, and level of care requirements.
  • May assist with precepting and mentoring new hospice aides, providing role modeling, reinforcing proper performance of tasks, safety practices, and hospice philosophy, and offering real-time guidance and constructive feedback.
  • Communicates effectively with patients, families, and team members.
  • Reports patient-related information and problems in a timely manner.
  • Documents observations and tasks performed accurately in the medical record.
  • Works in collaboration with all members of the health care team.
  • Utilizes chain of command appropriately.
  • Answers phone and patient calls.
  • Prioritizes work and responds appropriately to emergency situations.
  • Uses emergency systems, policies, and procedures.
  • Performs computer down time procedures according to unit/TriHealth guidelines.
  • Maintains a safe environment, follows patient fall prevention guidelines, and reports safety hazards.
  • Complies with Infection Control policies.
  • Demonstrates knowledge of safety/emergency policies and procedures.
  • Places emergency calls timely as directed.
  • Maintains confidentiality.
  • Takes an active role in maintaining patient satisfaction.
  • Maintains proficient knowledge of computer applications specific to their unit/job.
  • Provides other clerical support to the unit as needed.
  • Maintains competency of tasks in which the hospice aide is trained.
  • Completes assigned tasks within the required timeframe.
  • Seeks clarification from the nurse when in doubt about how to proceed.
  • Recognizes and reports problems with supply/equipment availability in a timely manner.
  • Reports and follows through on equipment that is broken or malfunctioning.
  • Assures equipment/supplies are readily available for use by the next person.
  • Demonstrates/maintains knowledge of organizational and unit changes.
  • Supports organizational and unit changes.
