About The Position

The Hospice Aide provides compassionate personal care and assistance with activities of daily living (ADLs) while ensuring patient safety, comfort, and dignity while delivering exceptional customer service. This role supports the interdisciplinary care team by following the Plan of Care, documenting services accurately, communicating changes in patient condition, and participating in ongoing quality improvement and professional development.

Requirements

  • At least one year experience working with Hospice as Certified Nursing Aide or a Home Health Aide.
  • Certified Nurse Aide Certificate or Home Health Aide Certificate
  • Possession of a certificate of completion from a basic aide training program comprising a minimum of 75 hours of training.

Responsibilities

  • Provide personal care and assist with activities of daily living (ADLs) according to the Plan of Care (POC).
  • Maintain a safe environment for the patient.
  • Assist with safe ambulation and transfers as appropriate.
  • Perform passive and active range-of-motion (ROM) exercises as directed by the nurse.
  • Prepare appropriate nutritional snacks for the patient.
  • Under the direction of the nurse, provide basic wound care, skin care, and bowel care according to the Plan of Care.
  • Observe, recognize, and promptly report changes in the patient's physical or mental condition to the nurse.
  • Document care provided and patient observations in the clinical record and complete all required charting, time sheets, and mileage logs in accordance with agency policies and procedures.
  • Attend and actively participate in patient care conferences, discipline meetings, team meetings, staff meetings, retreats, in-services, and other assigned meetings.
  • Participate in the on-call rotation as scheduled.
  • Assist with chart reviews, quality improvement activities, and other assigned projects as directed by the supervising nurse or agency leadership, consistent with applicable scope of practice.
  • Coordinate and communicate with volunteer staff as appropriate.
  • Provide basic caregiver and family education, including turning and positioning, transfer techniques, and linen changes.
  • Complete all required annual competency assessments and in-service training.
  • Performs all other duties as required.
