About The Position

The Hospice Aide is responsible for delivering patient care services under the direction and supervision of a Registered Nurse, as outlined in the care plan.

Requirements

  • High School Diploma or equivalent.
  • Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
  • CNA experience within the last two (2) years. If it has been longer, you must complete another training and competency program before providing services.
  • Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
  • Successful completion of a competency evaluation prior to patient care.
  • TN: A person who has completed a total of seventy-five (75) hours of training which included sixteen (16) hours of clinical training prior to or during the first three (3) months of employment and who is qualified to provide basic services, including simple procedures as an extension of therapy services, personal care regarding nutritional needs, ambulation and exercise, and household services essential to care at home.

Responsibilities

  • Report to the Executive Director or designee after each patient encounter regarding the patient's condition or environment and following the Care Management process as required.
  • Assist with activities of daily living during each visit, including mobility, transfers, walking, grooming, bathing, dressing or undressing, eating, and toileting.
  • Perform household services necessary for the patient's health care at home, helping to prevent or postpone institutionalization.
  • Participating in the Quality Assessment & Performance Improvement (QAPI) process by identifying issues and reporting to the administrator as needed.
