About The Position

Hearth Hospice is seeking a full-time Hospice Aide (CNA) to provide hands-on care to patients receiving end-of-life care. Working closely with the nursing team, the aide helps meet patients’ physical, emotional, and personal needs with respect, empathy, and professionalism. The position covers Hamilton County and is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m. Hearth Hospice describes itself as a mission-driven, patient-centered provider of end-of-life care. It aims to support patients and loved ones with comfort, compassion, dignity, peace, and human connection. Requirements

High school diploma or GED.
Current, unencumbered CNA certificate in the state where the employee works.
At least one year of CNA experience in an acute care or long-term care setting.
Current CPR certification if required by the state.
Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

Follow patients’ plans of care to support their comfort and well-being, and respond promptly to requests from patients and their families or caregivers.
Assist patients with personal care, including bathing, oral hygiene, changing bedding and clothing, and shampooing.
Provide exceptional customer service and work to promote patient satisfaction.
Share knowledge and expertise with families, guiding and supporting them in caring for terminally ill loved ones.
Maintain accurate documentation in patients’ homes and in accordance with agency and regulatory standards.
