We are hiring an After Hours Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Cadillac, MI area. Requirements

• Active Licensed Practical Nurse (LPN) license in the state you work.
• Two years of LPN experience and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Legally authorized to work in the United States.

• Hospice experience preferred.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Provide after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate.
• Educate and support patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey.
• Ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
