About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. We are hiring an After Hours Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Cadillac area. The schedule will be Monday through Friday, 4pm-12am. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. You will provide after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate. Additionally, you will educate and support patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey. Thorough, compliant EMR documentation is required. The role requires working independently, but with team support, and traveling to patient homes within an assigned territory.

Requirements

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• Legally authorized to work in the United States
