As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families. Requirements

• Current State license as a Registered Nurse.
• Current driver’s license.
• Current CPR Certification.
• Two (2) years' experience as a Registered Nurse in a clinical care setting
• One (1) year full-time hospice preferred.
• Nursing skills as defined as generally accepted standards of practice.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.
• Excellent verbal and written communication skills.
• Good interpersonal skills.
• Reliable transportation and valid auto liability insurance.

Responsibilities

• Coordinates and oversees all aspects of new patient admissions
• Collaborates with patient care teams to ensure seamless communication
• Educates families on patient condition, needs, and care
• Performs head-to-toe physical assessments
• Timely and accurately completes documentation
• Other duties as assigned
