About The Position

The decision to begin hospice care can be overwhelming for patients and families. At Concho Hearts Hospice, our Admissions Nurses help make that transition a little easier. We're looking for a compassionate and confident Registered Nurse (RN) to join our Kilgore team as a Hospice Admissions Nurse. This is a great opportunity for an RN who enjoys patient interaction, education, clinical assessment, and helping families understand what hospice care can provide.

Requirements

• Active Texas Registered Nurse (RN) license.
• Strong nursing assessment and communication skills.
• Comfortable educating patients and families about end-of-life care.
• Organized and able to manage admission documentation efficiently.
• Compassionate, dependable, and confident working independently.
• Reliable transportation, valid driver's license, and current auto insurance.
• Current CPR certification.

Nice To Haves

• Hospice or home health experience preferred.

Responsibilities

• Complete hospice admission visits in patients' homes, hospitals, facilities, and other care settings.
• Perform comprehensive nursing assessments.
• Educate patients and families about hospice services, medications, equipment, and what to expect.
• Review and complete admission documentation and required consents.
• Collaborate with physicians and the hospice interdisciplinary team.
• Coordinate medications, DME, supplies, and other immediate patient needs.
• Help ensure a smooth transition into hospice services.
• Communicate important patient information to the RN Case Manager and clinical team.
• Complete accurate and timely documentation in the EMR.
• Provide reassurance and compassionate support to patients and families throughout the admission process.
