About The Position

Salus Hospice is urgently seeking a compassionate and skilled Hospice Admissions RN to join our dedicated team serving the Orange County region. This full-time or per diem position offers the opportunity to make a difference in patients' lives by providing exceptional hospice care in a collaborative and supportive environment. The role involves managing the admissions process, evaluating patient appropriateness for hospice, building referral relationships, and educating patients and families about hospice care.

Requirements

Registered Nurse with valid CA state license
Minimum one year of hospice or home health experience
Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patient’s place of residence
Must have excellent interpersonal skills and a passion for hospice
Understanding of hospice philosophy and needs of the terminally ill
Self-directed with ability to work with little supervision
Current Physical and TB test results
VIETNAMESE or KOREAN fluency

Nice To Haves

Strong background in medical/surgical, ICU/CCU, or ER is helpful

Responsibilities

Participation in the information gathering, analysis, education and form completion aspects of the admissions process
Evaluate patients for hospice appropriateness, e.g. medical records review as requested.
Obtain agreement for admission of referred patients and families who meet admission criteria.
Build effective referral source relationships to support the high quality and responsiveness expectations associated with Salus Hospice
Maintain clinical and professional competence.
Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice care.
