Hospice Admissions RN
About The Position
As an admissions nurse at VITAS Healthcare, you are often the first face of hospice for patients and families and may lead their first in-depth discussion about end-of-life care. Your approach helps set expectations for the journey ahead. You travel within an assigned service area to meet patients and families, assess patients’ needs for care, explain hospice services, and help determine whether hospice is the right path. You work alongside a team to support a comfortable transition for patients who elect VITAS hospice care, with compassionate, personalized care that honors each patient’s wishes. Most admissions nurses work four 10-hour shifts per week, although schedules may vary. Work involves traveling to hospitals, nursing facilities, and patient residences as calls arrive.
Requirements
- Current RN license.
- Reliable transportation.
- Previous experience in a physician’s office, hospital, long-term care facility, or assisted living facility.
Nice To Haves
- Bachelor’s degree preferred.
Responsibilities
- Travel to meet with patients and families.
- Educate patients and families about the benefits of hospice care and the services offered by VITAS.
- Ask questions and listen empathetically to understand each patient’s end-of-life needs, wants, and beliefs.
- Use information gathered to determine whether hospice is the right path for the patient.
- Help ensure a comfortable transition for patients who elect hospice care with VITAS.
- Travel within the assigned service area to assess patients’ needs for care at hospitals, nursing facilities, and patient residences.
