About The Position

Houston Hospice is recruiting for Full-time, Part-time and PRN Admission RNs. Since 1980, Houston Hospice has grown to become the oldest and largest independent nonprofit hospice in the Greater Houston area. Our mission is to provide uncompromising, compassionate, end-of-life care to patients of all ages and their families. We serve the 13 counties surrounding the Houston area and are committed to being the highest quality hospice provider. Our approach is grounded in teamwork and trust, bringing together skilled healthcare professionals to provide comfort, dignity, and peace to patients. Care can be provided in a patient's home, a long-term care facility, or our IPU in the Texas Medical Center. Our team is dedicated to meeting patients where they are, both physically and emotionally. Our doctors and nurses are experts in easing pain and managing complex symptoms of life-limiting illnesses, allowing patients and families to focus on what matters most: being together.

Requirements

  • Graduate of an accredited school of nursing with a current Texas license as a Registered Nurse.
  • Current certification in basic life support.
  • Excellent organizational skills to handle a variety of tasks simultaneously.
  • Effective interpersonal skills.
  • Excellent communication skills, both oral and written.
  • Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
  • Ability to function as part of an inter-disciplinary team.
  • Ability to travel to multi-work locations during workday.
  • Must have reliable, insured vehicle and valid Texas Driver’s license.

Responsibilities

  • Provide full-cycle admission process to potential patients and families.
  • Provide hospice education.
  • Obtain consent documentation.
  • Perform comprehensive nursing assessment.
  • Develop and implement initial plan of care.
  • Communicate with staff, physicians, referral sources and prospective clients regarding hospice services and appropriate admission criteria.
  • Handle initial physical and emotional assessment of the patient/family upon admission and reviews the hospice concept with them.
  • Develop a detailed, written POC for each patient admitted in conjunction with the IDG, patient, family and attending physician.
  • Explain to patient and family what items hospice pays including a review of the medications list.
  • Instruct patient/family about appropriate techniques for care of the patient.
  • Complete all consents and legal documents accurately and concisely.
  • Communicate with PCM, patient DME, HHS and medication needs.
  • Establish the Home Chart with appropriate documents.
  • Communicate to the IDT report on new admissions and prioritizes needs.
  • Participate in orientation of staff and continued education of medical students, nursing students, and other health care professionals including community as appropriate.
  • Assist with routine visits if no admissions are scheduled for the day.
  • Document professionally, accurately, timely and according to established standards.
  • Understand and adhere to organizational privacy practices and compliance programs.
  • Assist in providing service that fosters the organization’s mission, values, vision and goals.
  • Perform other job related duties as required and/or requested by management.
