About The Position

James River Hospice is seeking a compassionate Hospice Admissions/Resource Registered Nurse to join its team. The role evaluates patients for hospice eligibility, coordinates admissions, and supports patients and families as they transition into hospice care. The nurse also provides care to General Inpatient Hospice (GIP) patients in hospital settings and supports community hospice patients as needed, working with the interdisciplinary team to provide patient-centered end-of-life care.

The schedule is Monday through Thursday, 9:00 AM to 7:00 PM. The position offers a salary of $90,000 to $100,000, based on experience.

Requirements

• Current Registered Nurse license to practice in the Commonwealth of Virginia.
• At least one year of hospice experience.
• Graduation from an accredited nursing school.
• Valid driver's license and a reliable car available for use during work hours.

Responsibilities

• Complete hospice admissions and evaluate patients for hospice eligibility.
• Coordinate care and admissions to support smooth transitions into hospice services.
• Assess patient needs and develop individualized care plans.
• Educate patients and families about hospice services and end-of-life care.
• Provide care to GIP hospice patients in hospital settings.
• Support community hospice patients through visits, pain and symptom management, and other clinical needs as required.
• Manage medications, complete documentation, coordinate care, and advocate for patients.
