About The Position

As a Bluegrass Care Navigators' Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers. Requirements

  • Graduate of an accredited school of registered nursing.
  • Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing.
  • Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations.
  • Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity.
  • Adheres to accepted professional, ethical, and legal standards of nursing practice.
  • Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying.
  • Maintains current CPR certification.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.

Nice To Haves

  • Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered.

Responsibilities

  • Serves as the first point of clinical contact for patients and families entering hospice home care.
  • Assesses patient eligibility for hospice home care.
  • Completes comprehensive nursing assessments.
  • Initiates individualized care plans in accordance with hospice guidelines.
  • Educates patients and families about hospice philosophy, services, and care processes.
  • Ensures compassionate, patient-centered support during a critical transition.
  • Collaborates with the interdisciplinary team, physicians, and referral sources to facilitate timely and seamless admissions.
  • Ensures regulatory compliance and the highest standards of care.
