About The Position
RN Admissions - Hospice Location: Inland Empire Schedule: Full-Time | Field-Based Pay: $62–$67/hour Join Cabrillo Hospice Cabrillo Hospice provides high-quality, patient-centered end-of-life care. Our team supports patients and their families with compassion, dignity, and strong clinical care during one of the most important times in their lives. We are seeking an experienced RN Admissions Nurse who is confident, compassionate, and comfortable working in the field. Role Overview As an RN Admissions Nurse, you will be the first clinical point of contact for patients and families entering hospice services. You will conduct assessments, guide the admission process, and ensure a smooth transition into care.
Requirements
- Graduate of an accredited nursing program
- Active California RN license in good standing
- Minimum 1 year of hospice experience required
- Strong assessment and clinical decision-making skills
- Excellent communication and patient education skills
- Valid driver’s license, reliable transportation, and auto insurance
Nice To Haves
- Bilingual Spanish and English is a plus
Responsibilities
- Conduct in-home hospice admission assessments and develop initial plans of care
- Collaborate with physicians to confirm eligibility and coordinate admissions
- Educate patients and families on hospice services and care expectations
- Complete accurate and timely EMR documentation within 24 hours
- Travel throughout Orange County and Inland Empire
- Work closely with the interdisciplinary team to ensure continuity of care
