About The Position

Ascension at Home together with Compassus is seeking a Hospice Admissions Registered Nurse. The Hospice Admission Registered Nurse will assess patients for hospice eligibility, explain hospice care services to patients and families/caregivers, ensure all initial patient needs are met, and collaborate with the Inter-Disciplinary Team regarding the plan of care and patient condition.

Requirements

• RN license in the state you work
• Two years of experience as a registered nurse, hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification

Responsibilities

• Assess patients for hospice eligibility
• Explain hospice care services to patients, families/caregivers
• Ensure all patient’s initial needs are addressed
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information
