Hospice Admissions Education RN Specialist
About The Position
Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated RN to join our team in Admissions as an Admissions Education Specialist Nurse in the Broward County area while providing exceptional care on the following schedule: 9am - 7:30pm. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.
Requirements
- Registered Nurse currently licensed in the State of Florida.
- Current Basic Life Support Certification.
- Strong and current nursing clinical skills required.
- Expertise in home health, case management, hospice/palliative care, eligibility for services, pain and symptom management.
- Computer expertise and EMR documentation expertise, including an understanding of critical thinking and care planning process.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
Nice To Haves
- BSN preferred.
- A minimum of 2 years in a supervisory/management role, clinical preceptor, manager or mentor in a hospice setting preferred - OR - an equivalent combination of education, training and experience.
- Hospice certification preferred.
Responsibilities
- Train, coach, and support Admissions and Business Development team members throughout the hospice admissions process.
- Ensure new and transiting staff development competency, confidence and clinical excellence when initiating patient care.
- Design and deliver individualized onboarding and education plans using adult learning principles.
- Serve as a clinical and EMR resource, providing bedside teaching, documentation guidance and real-time problem solving.
- Collaborate with Organizational Development, leadership and interdisciplinary teams to ensure regulatory compliance and high-quality admissions education.
