About The Position

The Hospice Admissions Coordinator (Intake Coordinator) manages the referral-to-admission process for hospice patients. This role is responsible for coordinating hospice referrals, insurance verification, physician documentation, and admission scheduling to ensure patients are admitted efficiently and in compliance with Medicare, payer, and hospice regulatory guidelines. The Admissions Coordinator works closely with Business Development Specialists, physicians, clinical teams, patients, and families to confirm referral information, obtain required documentation, verify insurance eligibility and hospice benefits, and coordinate scheduling for patient admission. This position plays a critical role in maintaining an efficient and compliant hospice intake and admissions workflow.

Requirements

  • Current Licensed Vocational Nurse (LVN) license in the state of practice required.
  • High school diploma or equivalent required
  • Minimum 1–2 years of experience in hospice admissions, hospice intake, healthcare admissions coordination, medical scheduling, or patient intake within a healthcare setting.
  • Experience verifying insurance eligibility and benefits for Medicare, Medicaid, and/or commercial insurance plans.
  • Experience working within a healthcare EMR system
  • Strong organizational skills with the ability to manage multiple patient referrals simultaneously in a fast-paced healthcare environment.
  • Excellent communication skills when working with physicians, hospitals, patients, families, caregivers, and clinical staff.
  • Strong computer proficiency including EMR documentation systems, data entry, and healthcare workflow tools.

Nice To Haves

  • additional healthcare administration or related education preferred.
  • hospice EMR experience strongly preferred.
  • Familiarity with Homecare Homebase or a similar hospice documentation system preferred.
  • Knowledge of Medicare hospice requirements, physician documentation, hospice eligibility, and certification requirements preferred.
  • Experience coordinating with hospice nurses, social workers, aides, chaplains, physicians, and referral partners preferred.

Responsibilities

  • Manage incoming hospice referrals and initiate the internal admissions process.
  • Review referral documentation to ensure required clinical records and supporting documentation are present for hospice eligibility and admission.
  • Communicate with hospitals, physician offices, case managers, facilities, and referral partners to obtain missing documentation required for admission.
  • Enter and maintain accurate patient demographics, referral details, clinical information, and documentation within the EMR system.
  • Perform insurance verification, eligibility review, hospice benefit verification, and authorization coordination for Medicare, Medicaid, and commercial payers as applicable.
  • Coordinate with clinical leadership, hospice nurses, physicians, and scheduling teams to arrange timely patient admissions.
  • Monitor referral status and ensure admissions are completed timely and in accordance with regulatory and organizational requirements.
  • Communicate referral updates with Business Development Specialists, intake staff, clinical teams, and operational leadership.
  • Contact patients and families or caregivers to confirm demographic information, verify insurance coverage, and explain the hospice admissions process.
  • Maintain admissions logs, referral trackers, and intake dashboards to monitor referral activity and escalate delays when necessary.
  • Ensure referral and admission documentation meets Medicare, Medicaid, payer, and hospice compliance standards.
  • Support a smooth transition from referral to hospice admission by maintaining clear communication between referral sources, patients and families, and the interdisciplinary hospice team.
