About The Position

JourneyCare Hospice is seeking a Hospice Admission RN. This role offers new competitive salaries, an immediate opening, generous time off packages, 401K with match, and more. The position is for a FLOAT RN covering Barrington, Palatine, and Rolling Meadows, IL, with working hours of Monday-Friday, 4 PM-12:30 AM, including a holiday rotation. The role involves performing pre-admission evaluations, initiating the admission process, obtaining informed consent, and providing nursing assessments, initial plans of care, and interventions to optimize patient and family-directed comfort and quality of life.

Requirements

Graduate from an accredited registered nursing program
Licensed as a registered nurse in the state of practice
Must possess current CPR Certification
Must possess the ability to make independent decisions when circumstances warrant
Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public
Must be knowledgeable of quality assessment and assurance procedures

Nice To Haves

1 year of Hospice experience, preferred

Responsibilities

Perform pre-admission evaluations for the appropriateness of services.
Initiates the admission process by educating and informing patients/families of its services.
Obtains informed consent.
Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
