This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Requirements
- Willing to travel throughout the Bay Area.
- Comfort with travel is essential.
- Minimum 1 year of hospice experience, SNF, or hospital experience.
- Willingness to travel to meet our patients where they live (private homes, assisted living facilities, hospitals, etc.).
- Reliable transportation, valid driver’s license, and auto insurance.
- Registered Nursing Licensure in the state of California.
Responsibilities
- Complete hospice admissions and patient visits as needed.
- Orient new patients to the hospice program.
- Assist the patient and caregiver with all the legal paperwork.
- Complete a thorough nursing assessment.
- Establish hospice eligibility and diagnosis when patient is appropriate.
