Requirements

Graduate of an approved nursing education program
Current Registered Nurse (RN) license in the state of Ohio
Minimum of one (1) year of acute care nursing experience
Strong communication and interpersonal skills, including grief and loss support
Valid driver’s license and reliable transportation
Commitment to Trinity Health’s Mission, Values, and Code of Ethics

Nice To Haves

Hospice or home health experience preferred

Responsibilities

Conduct holistic assessments addressing physical, emotional, psychosocial, and spiritual needs
Educate patients and families about hospice philosophy, services, and care expectations
Coordinate hospice admissions, consents, and transitions of care
Establish and communicate the initial hospice plan of care
Collaborate with physicians, interdisciplinary teams, and hospice leadership
Provide functional supervision to LPNs and Hospice Aides
Document accurately and in a timely manner to support continuity and quality of care
Participate in admissions and on‑call rotations as assigned
