About The Position

The Hospice Admissions Nurse assesses patients in their place of residence or in hospitals and prepares the patient for entry into the spectrum of services provided by Gilchrist Hospice Care. The Hospice Admissions Nurse also serves as an ambassador for Gilchrist Hospice Care within the service area. Gilchrist Hospice Care is Maryland's premier nonprofit provider for serious illness and end-of-life care, dedicated to ensuring every moment is lived to its fullest. This care is provided wherever the individual designates as their home, including private residences, nursing homes, or assisted living facilities, and is available to patients of any age, religion, race, or illness, covered under Medicare, Medicaid, and most private insurance plans.

Requirements

• Diploma, ADN and/or BSN from an accredited institution
• Current MD nursing license
• Three years of nursing experience
• Knowledgeable of universal precautions for infection control and practices safe nursing standards for self protection
• Strong time management skills and the ability to function with a changing daily schedule
• Excellent interpersonal and communication skills
• Ability to be patient and family focused and provide patient/caregiver education
• Skill in the use of EMR (Electronic Medical Record)
