About The Position

We are looking for a proven, people-first leader with experience implementing community health initiatives and managing teams in community-based or nonprofit settings to take on a pivotal leadership role in the success of our Horizon Program. Requirements

  • Demonstrated experience executing workplans in a nonprofit, healthcare or community-based setting.
  • Background in public health, grants management, community programming
  • Experience with grant writing and program reporting
  • Strong understanding of healthcare, social services, and public benefits systems
  • Demonstrated experience managing teams in a nonprofit, healthcare, or community-based setting.
  • Familiarity with community resources and networks in Central Maine
  • Proficiency in using data to drive decision-making and improve outcomes
  • Excellent communication and interpersonal skills
  • Bachelor’s degree in Public Health (PH), related field, or equivalent experience preferred.

Responsibilities

  • Implement community health initiatives within a rural health service area
  • Track and analyze performance data to ensure strong patient outcomes
  • Manage project budgets and basic accounting for program expenses
  • Supervise and mentor staff
  • Facilitate collaboration and linkages to care with clinical care teams and community organizations
  • Facilitate team meetings and educational opportunities
  • Facilitate patient support groups
