About The Position

The Homemaker (Per Diem) at VNA of Cape Cod is responsible for providing continuous companionship to patients and assisting with their daily living needs. This includes helping with meal planning, purchasing food, and preparing meals. A key aspect of the role is maintaining the patient’s living environment to ensure it is clean and safe. The Homemaker is also expected to observe and report any significant changes in the patient’s general physical, emotional, and mental conditions to the Clinical Liaison or Manager, thereby providing a sense of security for the patient. The role involves assisting patients with various needs, strictly excluding any personal or medical care. Accurate and timely completion of all required documentation, as per agency policy, is essential. Additionally, eligible Homemakers may have the opportunity to participate in the Patient Transportation program for the Private Services department. The position requires a consistent commitment to service excellence, reflecting VNA’s core values of compassion, accountability, respect, excellence, and service (CARES) towards all patients, family members, visitors, volunteers, and co-workers.

Requirements

  • Minimum six months of satisfactory employment history.
  • Satisfactory completion of Skill Assessment Test.
  • Able to understand and carry out direction and instruction.

Responsibilities

  • Provide continuous companionship to the patient while on duty.
  • Assist in meal planning, purchase of foods and food preparation.
  • Maintain the patient’s living environment in a clean and safe manner.
  • Observe the patient’s general physical, emotional and mental conditions and report any obvious changes to the Clinical Liaison or Manager.
  • Provide a sense of security for the patient.
  • Assist patients as needed, excluding any personal or medical care.
  • Complete all required documentation and submit such on a timely basis and as directed through agency policy.
  • If eligible, may participate in the Patient Transportation program for the Private Services department.
  • All other duties as assigned.
  • Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers in a manner that reflects VNA’s commitment to CARES: compassion, accountability, respect, excellence and service
