HOMECARE REGISTERED NURSE
About The Position
Care for our community, and your career. This role involves planning, coordinating, providing, and documenting the care of patients in their place of residence within Jefferson County. The primary goal is to ensure compassionate, engaged care that meets or exceeds quality standards, provided in an efficient and safe manner consistent with NYS licensure and Samaritan Home Health standards. The nurse will utilize nursing skills to assess patient needs, develop and revise care plans, observe patient progress, and implement treatment plans. This role also includes direct supervision of LPNs and HHAs, and providing on-call services as required.
Requirements
- Graduate of an accredited school of nursing
- Current NYS RN License required.
- Minimum 1 year of medical-surgical experience.
- BLS required.
- Valid NYS Driver's license and proof of auto insurance.
Nice To Haves
- Previous RN experience is preferred. New Graduate RN's welcome to apply.
- Previous Home Health Care experience preferred.
- ACLS preferred.
Responsibilities
- Planning, coordinating, providing and documenting the care of patients in their place of residence.
- Clinical practitioner who coordinates and implements patient care specific to Jefferson County.
- Ensures that compassionate, engaged care meets or exceeds quality standards.
- Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.
- Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care.
- Performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.
- Observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.
- Recognizes their responsibilities as specified by law and abides within those responsibilities.
- Provides direct supervision to LPNs and HHAs.
- Provides on-call services as required.
- Must be available weekends and Holidays and provide on-call services as required.
