Requirements

Valid Registered Nurse (RN) license in the state of Michigan.
Minimum of 2 years’ experience in nursing, preferably in home health or related settings.
Strong clinical skills with the ability to work independently.
Excellent communication and interpersonal skills.
Ability to manage time effectively and prioritize patient needs.
Reliable transportation to visit patients’ homes throughout Detroit.
CPR and BLS certification preferred.

Responsibilities

Conduct assessments of patients’ health conditions and develop individualized care plans.
Administer medications, treatments, and therapies as prescribed by physicians.
Monitor patient progress and update care plans accordingly.
Provide patient and family education about health management and disease prevention.
Collaborate with interdisciplinary teams to ensure coordinated and comprehensive care.
Document patient care accurately and timely in compliance with regulatory standards.
Respond to medical emergencies and provide appropriate interventions.
