Requirements

Active LPN or RN License issued by NYS
Minimum of 1 year experience in a homecare setting.
Current BLS Certification
Empathy, Reliability, and a commitment to high-quality care
Experience with trach care and suctioning.

Nice To Haves

Experience with ventilator-dependent patients
G-tube care
Seizure management
Medication administration

Responsibilities

Perform physical and Health Assessments before making critical decisions.
Provide health promotion, counseling, and education.
Administer medications and other personalized interventions.
Coordinate care, in collaboration with a wide array of health care professionals.
Document nursing notes regarding patient care daily.
Must be able to lift 50lbs.
Must be able to stand/work for extended periods.
Additional responsibilities as required.
